ENGINEERING NOTICE – EA-RE-02 - READING EAST TRAIN MOVEMENT ENGINEERING ALERT

BACKGROUND

During works for SRSA at Reading East on the 05/04/2025, an unintended train movement took place. Engineering train 6C21 came into the worksite with a double ended loco spoil and panel train with the intention of splitting it into two separate trains – Loco 1 country end and Loco 2 London end.

Site Supervisor made a mistake when he moved country end Loco 1 instead of Loco 2 on London end. Once train driver realised the supervisor wasn't intentionally talking to him, he applied the emergency brake and sounded horn. Train driver then exited the cab of the train and verified no personnel or plant were at risk. The engineering train did not move a significant distance. Supervisor confirmed there was no one behind the train at the time of the unintended movement.

IMMEDIATE ACTIONS:

    The following actions shall be taken on all SRSA worksites to mitigate the risk of this occurring again;

    During planning where trains are going to be split it must be clearly documented and briefed in whiteboards how these are going to be identified on site. The following options are available:

  • Give the two trains separate head codes and brief these to the drivers on site.
  • Identify the trains by Loco number
  • Use recognisable landmarks on each end of the train and brief them to drivers and supervisors prior to communicating movements.
  • Use head code – A (alpha) for leading Loco and head code – B (bravo) for trailing loco and clearly communicate these to the drivers and supervisors.
  • Ensure the two drivers are on separate radio channels and brief them to all supervisors in charge of train movements.
  • Only staff who have attended ‘Designated Person in Charge of Train Movements’ briefing are allowed to be in charge of train movements.
  • All persons in charge of train movements must confirm which loco driver they are communicating to before authorising train movements.
  • The person in charge of train movements shall stand in a position of safety and visually check that all staff have stood back and are clear of the exclusion zone before authorising train movements. Where this is not possible due to line-of-sight obstruction (e.g. on a curve, plant or the other side of the engineering train) the person in charge of train movements shall send a representative to check and confirm that all staff have stood back and are clear of the engineering train
  • Exclusion zones around Engineering Trains, OTM & OTP to be strictly maintained at all times.
