Due to the IPs difficulty releasing the bag frame from the Gimble hook, the operator lowered the arm of the RRV to aid him.The movement had not been requested by the IP, this resulted in the IPs finger becoming trapped between the top of the lifting eye and the hook.Key learning outcomes.Ensure all movements with plant are agreed and confirmed by the crane controller or machine controller first. Be certain via clear communication from the Crane Controller that the area is clear and also visually inspect that the lift can be conducted safely. The crane controller must ensure that the machine operator has received the instruction and communicates back to the crane controller that he understands the instruction via Dect Comms.Be aware of pinch points and ensure that all hands are clear of any areas when lifting or lowering and that suitable hand protection is worn at all times.Download Bulletin Name*FirstLast Email* Date* Choose A Response *Select valueI confirm that I have read and understood the bulletinI don't understand the bulletin and require more informationSubmitReset
