    (0011) Document Briefing

    The following Policies/Procedures are detailed within this briefing:

  • For Cause Screening Procedure
  • Just Culture Policy
  • Accident/Incident/Environmental Spillage Reporting & Investigation Procedure
  • Line Managers shall brief their team as soon as possible on the relevant information contained within these documents, as required.

    Once briefed, all individuals shall then record read & understood receipt through the internal briefing process.



For Cause Screening Process (PP093 V01)

    This procedure has been included into the ECS Integrated Management System.

    The purpose of this procedure is to describe the arrangements for alcohol & drugs screening of ECS employees & its contractors following an accident or incident or those whose conduct/behaviours or appearance gives concern for their health & safety or welfare of themselves or others.

    This procedure applies to both safety critical and non-safety critical employees and includes work away from the track/lineside boundary.

    The procedure places responsibilities on the following:

  • Responsible Managers/Head of Safety & Assurance – liaise with the company Directors about disciplinary action and assumes the role of “Person in Charge” who is authorised to summon screening.
  • Office Manager/Administrator – are to keep copies of all test results.
  • The procedure provides examples of when a For Cause screening must be carried out, such as following an initial report of an event that gives ground for suspecting that a person involved in the incident/accident is under the influence of drugs or alcohol.

    The procedure also details that the suspected individual must be prevented from carrying out any activity or duty, escorted to a safe working area and continuously monitored until the test sample collection has been completed.




    Just Culture Policy (POL062 V01)

    This procedure has been included into the ECS Integrated Management System.

    The purpose of this policy is to state the commitment of ECS and how it investigates and learns from previous events to support in continuous improvement.

    The policy states that it is a requirement for ECS to investigate all accidents and significant incidents to ascertain the facts and does not instigate the disciplinary procedure in the event of genuine lapses or mistakes.

    However, where it has been found that an ECS employee or contractor has knowingly and deliberately ignored company processes, procedures or Network Rail Rules and Regulations, the individual will be subject to the disciplinary process.

    The Policy also includes reference to the internal Behaviour Awareness Programme that ECS will be delivering across the business. The Behaviour Awareness Presentation is to provide education on human behaviours/factors (including habits, personal risk perceptions, time verses risk and communication) and why accidents/incidents happen and what measures can be taken to eliminate/reduce these events.




    Accident/Incident/Environmental Spillage Reporting & Investigation (PP002 V01C)

    This procedure has been included into the ECS Integrated Management System.

    The purpose of this procedure is to demonstrate a documented system for the reporting and investigating, as well as taking corrective/preventive actions in the event of an accident or incident involving ECS personnel, its contractors or member of the public.

    This procedure applies where an accident/incident occurred from ECS’ work activities.

    The procedure places responsibilities on the following:

    The Managing Director:

  • Provide assurance that arrangements are in place to carry out the investigation
  • Ensure that appropriate corrective and preventive actions are taken following an event.
  • Ensure that all RIDDOR events have been reported to the relevant enforcing authorities.
  • Issue Detailed Level Investigation Remit, when acting in the capacity of Designated Competent Person.
  • Contract Managers/Supervisors:

  • Brief their workforce on this procedure prior to work taking place
  • Ensure that all accidents/incidents/spillages/close-calls are reported in accordance with the Just Culture Policy.
  • Use the initial investigation report form and is complete/submit within 24 hours of the event.
  • When acting as a POS Representative, in addition to reporting any accidents/incidents, these shall also be reported to NIROnline.
  • Head of Safety & Assurance:

  • Review all initial accident/incident reports and discuss within the directors.
  • Ensure that the accident book is completed.
  • Report all RIDDOR events to the Statutory Authority in consultation with the Company Directors.
  • Investigate accident/incidents/spillages when appointed as Lead Investigator in accordance with the Detailed Level Investigation Remit.
  • Produce Detailed Level Investigation Reports as required when appointed as Lead Investigator
  • Employees & Subcontractors:

  • Report all accidents/incidents/spillages/close-calls to their Contract Manager/Supervisor.
